What to Bring to Your Appointment

Having the right documents on hand ensures your visit goes smoothly and your claim can be processed without delay.

  • Your insurance card The physical card or a screenshot of your digital benefit card.
  • Your plan member number Usually found on your insurance card or in your benefits portal.
  • A referral letter (if required by your plan) Not all plans require one — check with your provider beforehand.
